- Landowner Consent Form (PDF, 29kb) - Hunters wishing to hunt deer on private land on Barrie Island, Cockburn Island and Manitoulin Island (WMU 43A, 43B and 44), or moose on private land in WMU 65, are required to carry an MNR Landowner Consent form signed by the landowner.
- Letter of Consent for Hunter Apprenticeship Safety Card Applicants Aged 12-15 (PDF, 16kb) - Young people, aged 12 to 15, who wish to enroll in the Hunter Apprenticeship Safety Program must use this form to satisfy the written permission requirement.
- Letter of Consent for a Hunting/Fishing Outdoors Card and Related Hunting Licences for Applicants Age 15 Only (PDF, 16kb) - Fifteen year olds who wish to apply for a hunting version Outdoors Card must use this form to satisfy the written permission requirement.
- Ontario Hunter Education Program Consent Form (PDF, 61kb) - Effective January 28, 2008, those under 16 years of age wishing to participate in a Hunter Education or Turkey Hunter Education Course, must have written permission of a parent or guardian. This form is provided for your convenience. Prior to taking a course, the completed form must be given to the Hunter Education instructor, or to OFAH staff on the day of the Turkey Hunter Education Course.
